Nomination Process
Who is Eligible?
If you work with a staff member, volunteer or on-staff physician, dentist or midwife who demonstrates the four attributes: inspirational service; professionalism; integrity; and teamwork, nominate him or her. The Martha Curgin Inspiration Award is for anyone who works or volunteers at LHSC, LHSF, Lawson, CH or CHF. All previous nominees and recipients can be nominated again.
Who May Submit a Nomination?
Any staff member, physician, dentist, midwife or volunteer at LHSC, LHSF, Lawson, CH or CHF may submit a nomination form for consideration. A nominator can nominate more than one person.
How Will the Recipient be Celebrated?
The recipient will receive a framed professional photograph of themself along with a commemorative pin at a special award ceremony in June 2012. Recipients are also recognized on a permanent display within LHSC.
2012 NOMINATIONS ARE NOW CLOSED
1. IMPORTANT: Only online nominations will be accepted. Submissions which do not use the official nomination form will not be considered. Upon submission of the online form, both nominators will receive a confirmation email.
2. In your submission, please provide examples why you feel this individual deserves the Martha Curgin Inspiration Award. Make sure you include specific examples of how the nominee has met the selection criteria for the award in the following areas: inspirational service, professionalism, integrity and teamwork. Please note that nominators MUST limit their submission to 250 words in each of the four categories. Any additional information (such as letters of reference, etc.) will NOT be reviewed by the Selection Committee or included in the decision-making process.
Deadline for Nominations
Nominations for the 2012 Martha Curgin Award are now closed. The recipient and nominators will be informed in April, 2012.

Hints for Completing the Nomination Form
Some questions to answer within your examples:
1. How does this person live out their values?
2. How does this person demonstrate selflessness?
3. What is the impact this person has on patients, co-workers, families?
4. How does this person go above and beyond the expectations of their normal role?
A guide for writing an example:
Instead of writing: "Elizabeth is always helpful" explain
"how" by writing something like:
"An example of Elizabeth's service-oriented approach is when, after a long day in Surgery, she took time to assist students with their research project. Despite her long day and the many hours in the OR, she took several hours to ensure the students were well-oriented, knew what they were doing and had all the materials they needed. Her selfless approach, professionalism and interest in others is inspirational!"
Selection Process
Each nomination will be reviewed by the Selection Committee consisting of members from Senior Leadership from LHSC, LHSF, Lawson, CH or CHF; LHSC and LHSF Board of Directors; and members of Martha Curgin's family.
The Selection Committee is governed by terms of reference as approved by LHSF Board of Directors.
The Martha Curgin Inspiration Award recognizes ordinary individuals - staff members, physicians, dentists, midwives or volunteers - who provide extraordinary service to London Health Sciences Centre (LHSC).
