How to Make a Memorial Gift

To make a memorial donation online:

To purchase specific equipment or support a specific area of the hospital to create a special memory, visit our online catalogue.

To make a donation by cheque, please address the envelope to:

c/o (program name)

London, ON N6C 2R6

For example, if the donation is being directed to the prostate cancer program, the envelope should be addressed to:

London Health Sciences Foundation
c/o Prostate Cancer Program
747 Baseline Road East
London, ON N6C 2R6

Please make all cheques payable to:

London Health Sciences Foundation

On the memo line of the cheque, please write the program name and the name of the person you are honouring with the donation.

For example, if you are donating $20 to the prostate cancer program in honour of John Smith, the cheque would look like this:

Note that tax receipts are issued for donations of $20 or more.
